We are currently seeking a Nurse Practitioner (NP) to complete our Care Coordination Team in San Antonio, whose primary functions are to assist in post-hospital house calls and in the prevention of hospital admissions, readmissions, and emergency room visits in order to meet organizational objectives.
The schedule for this position is Monday to Friday, 8:
00am to 5:
00pmno on-call.
Primary
Responsibilities:
Consults with and educates the patient/family regarding disease process, self-care techniques, prognosis, preventative care, and treatment; Performs and documents visits to patient/family based on patient/family needs and barriers; Communicates with our San Antonio participating clinics (primary care physician and other providers) and ACO Care Coordination Team, as necessary, regarding patients care and barriers; Informs physicians and other providers and or personnel of changes in the patient's needs and outcomes of intervention while evaluating; Able to manage time while being effective and efficient; Adheres to agency policies regarding protected health information (PHI); Provides education to clinic's care coordination team Licensure:
Current State license as a Registered Nurse and as an Advanced Practice Nurse; Current Driver's License
Experience:
Experience is preferred, but not required Skills:
Nursing skills as defined as generally accepted standards of practice; Comprehensive knowledge of pharmacology and acute care, preferred; Bilingual (English/Spanish) preferred; Transportation:
Reliable transportation; Valid and current auto liability insurance; Environmental and Working Conditions:
Works in patients' homes; Flexible work schedule; License/Certification:
Certified Nurse Practitioner Compensation:
