Provide initial consults and daily follow-ups for the duration of the patient's hospital stay as indicated.
Work directly with patients, families, and interdisciplinary team in a hospital inpatient setting.
Complete patient history and physical exam independently.
Review patient's chart and assess a care plan for the patient; communicate with family members or caregivers.
Discuss goals of care & advanced care planning with patient, family, and/or caregivers.
Be mindful and respectful of any emotional or spiritual needs of patient and family that can be addressed.
Order appropriate medications and alter existing treatments to address patient's pain and symptoms.
Coordinate discharge plan with Care Team and Case Management.
Close feedback loop and follow up with referring physician when indicated.
Prepare and maintain accurate and timely patient records and notes after patient encounter.
Submit appropriate codes for accurate and concise billing for patient visit.
Perform duties autonomously; coordinate with supervising physician as needed.
Liaison with internal Care Coordinators and transition to outpatient services after discharge for continuity of care.
Create positive relationships with hospital staff and administration to build and maintain a referral sources and effective communication.

ACNP, PA, or DNP or equivalent Active Palliative Care Certification(s) Active Texas & Federal Licenses required for medical practice 1-2 years of clinical experience Hospice or Palliative Care experience preferred, but not required Valid driver's license and auto liability insurance Must submit proof of vaccination status for COVID-19, in compliance with CMS requirements.
