Registered Nurse (Nurse Care Coordinator)

You will collaborate with internal interdisciplinary team members to initiate referrals and access medical resources. Interact with external fee providers and agencies, Department of Defense stakeholders and other community providers and agencies to identify and coordinate medical services not available through the South Texas Veterans Healthcare System. You will analyze data and coordinate with the network interdisciplinary team to develop a plan for the appropriate level of care that addresses the patients consult needs. Educate and inform patients on Community Care programs and guidelines and scheduled appointment times. Refer consults and make provisions for continuing care with healthcare providers. Apply knowledge of disease management and utilization review and take the leadership role in using safe, cost effective measures to direct patient consults. Procure and coordinate healthcare resources based on patient need, eligibility and availability. Document observations, assessments, interventions and outcomes in the patient's medical record. Identify problem areas for monitoring and evaluation and is active in analyzing findings and changing practice based on quality improvement findings. Serve as an educational resource and provide consultation to other staff on continuing care needs and identification of appropriate resources utilizing evidence based criteria. Use discretion in confidential matters and lead others in appropriately safeguarding confidential information. Work Schedule:

The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Experience:
Bachelor's Degree in Nursing (BSN) Recent five years of clinical nursing experience in the acute care setting of medical and surgical. Three years of experience in case management Strong, effective communication and customer service skills with demonstrated ability to function independently. Ability to serve as an effective advocate for patients. Ability to navigate complex health care systems. Demonstrated skill in managing multiple projects and priorities. Demonstrates excellent time-management skills. Knowledge of VA regulations, policies, and procedures, including eligibility criteria for Care in the Community. Proficient in use of VistA, CPRS, Microsoft Office, Microsoft Excel, etc. Physical Requirements:
The position requires light lifting and carrying (under 15 pounds), straight pulling (1 hour), reaching above shoulder, use of fingers, both hands required, walking (4 hours), standing (2-7 hours), kneeling (2 hours), repeated bending (2 hours), climbing (use of legs and arms), both legs required, ability for rapid mental and muscular coordination simultaneously, near vision correctable at 13 to 16 to Jaeger 1 to 4, specific visual requirement (20/40 DVA, corrected), both eyes required, ability to distinguish basic colors, hearing (aid permitted), sitting (8 hours), simple grasping (4-6 hours), fine manipulation (to include keyboarding 6-8 hours), twisting (1 hour), and mental and emotional stability.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
